This voucher is issued exclusively for the Cervical Screening Program provided by Health Screen – Makati Branch, in partnership with End Cervical Cancer Philippines Organization, Inc.
It is intended to support accessible and timely cervical cancer screening for eligible clients.

What the Voucher Covers

  • Inclusions: Physician’s professional fee

  • Location: Health Screen – Makati Branch only

This voucher applies strictly to cervical screening services. All other services are excluded.

How to Avail

  1. Schedule an appointment in advance by calling 0976.215.7793

  2. Present the original voucher at the Health Screen – Makati Branch on your appointment date.

  3. Surrender the voucher upon redemption.

Voucher Terms & Conditions

  1. This voucher is valid solely for cervical screening services under the Health Screen – Makati Branch and End Cervical Cancer Philippines Organization, Inc. program.

  2. The voucher applies only to cervical screening services regularly priced at ₱1,000, inclusive of the physician’s professional fee. All other services are excluded.

  3. This voucher may not be combined with any other promotion, discount, or package rate.

  4. Valid until January 28, 2026 only, with no extensions.

  5. One-time use per client only. Voucher must be surrendered upon redemption.

  6. Non-transferable and not convertible to cash, credit, or other services.

  7. Advance appointment is required.

  8. Lost, damaged, altered, photocopied, or digital copies of this voucher shall not be accepted or replaced.

  9. Health Screen and CerviQ reserve the right to refuse vouchers suspected of misuse, alteration, or tampering.

  10. Not valid for corporate accounts, HMO-covered services, or medical missions.

  11. Voucher use is subject to applicable laws, government regulations, and compliance requirements.
